First, you complete the online patient intake form on our home page.

 

Our form only takes a few minutes and gives our medical practitioners the information they need to complete your medical document.

After your form is submitted, you next step is to pay the $50 consultation fee.

 

This fee pays for your services at your clinic including the cost for medical practitioners to:

• Review your file

• Complete and submit your medical cannabis document

• Send you a medical cannabis treatment plan 

Within 2 business days of submitting your patient intake form and paying your consultation fee, you will get an email from our medical practitioners with the following information:​

• Your treatment plan for medical cannabis 

• Confirmation email that your medical document has been successfully submitted

You will get a confirmation email from your licensed producer within 1 to 2 weeks that your medical cannabis is ready to order
